EVV Made Simple

Electronic Visit Verification (EVV) is how you clock in and out of your visits. It takes less than a minute, and this page walks you through it — no tech skills needed.

The 6 things EVV records at every visit

Wisconsin Medicaid requires these six pieces of information for every visit. When you clock in and out correctly, all six are captured automatically — you don't have to write anything down.

  • Who receives the service

The client you are caring for. Always confirm you are checked in under the correct client's name.

  • Who provides the service

That's you. Always clock in with your own login — never let anyone else clock in for you.

  • What service is provided

The type of care you're giving, such as personal care or supportive home care.

  • Where the service is provided

The system records the location, usually the client's home, when you check in and out.

  • The date of service

The day the visit happens. Clock in on the same day — never backdate a visit.

  • Start and end time

The exact time you begin and end care. This is why clocking in and out on time matters so much.

Your visit, step by step

  1. Arrive and clock in right away

As soon as you arrive at the client's home, open the EVV app and clock in. Don't wait until you've settled in — the start time should match when care actually begins.

  2. Confirm the client and service

Check that you're clocked in under the right client and the right type of service before you start.

  3. Provide care as planned

Follow the client's care plan. The EVV record runs in the background while you focus on the person in front of you.

  4. Clock out before you leave

When the visit ends, clock out while you are still at the client's home. This records the correct end time and location.

  5. Fix mistakes the same day

Forgot to clock in or out? Tell the office right away. Missed punches can be corrected, but waiting makes it harder and can delay your pay.

Common mistakes to avoid

  • Clocking in from your car down the street or from home — the location won't match the client's address.
  • Waiting until the end of the shift (or the end of the week) to clock in for earlier visits.
  • Letting a coworker or family member clock in for you — this is never allowed.
  • Forgetting to clock out, which makes your hours wrong and holds up payroll.
  • Staying silent when something goes wrong — the office can fix mistakes, but only if you tell them quickly.

Question 1 of 5

When should you clock in for a visit?

EVV questions caregivers ask

What is EVV?

EVV stands for Electronic Visit Verification. It's a phone or tablet app (or a phone call system) that records when and where you start and finish care. Wisconsin Medicaid requires it for personal care and home health services — it's the law under the federal 21st Century Cures Act, not something Avida invented.

Why do I have to use it?

Medicaid requires proof that visits actually happened before it pays for care. Without a correct EVV record, Avida cannot bill for the visit — and that can delay your paycheck. EVV also protects you: it's proof you were there doing your job.

What if I forget to clock in or out?

Call or text the office at (414) 348-4600 as soon as you notice — the same day if possible. A missed punch can be corrected with a note explaining what happened, but repeated missed punches cause payroll delays, so make clocking in the first thing you do at every visit.

Does EVV track me all day?

No. The system only records your location at the moment you clock in and clock out. It does not follow you around or track you between visits.

What if the client doesn't have internet or a smartphone?

There are backup options, including clocking in by a phone call from the client's landline or a small device kept at the home. If you're having trouble at a client's home, call the office and we'll set up the right option.

Is my information private?

Yes. EVV information is encrypted and protected under HIPAA, the same law that protects client health information. Only Avida, the state, and the payer can see visit records.

I see 2 or 3 clients in one day — do I clock in for each one?

Yes. Every visit gets its own clock in and clock out — even two visits with the same client on the same day, or back-to-back clients. Never combine visits into one long entry; each time slot needs its own record so every client is covered and your hours are correct.

Official Wisconsin EVV resources

These come straight from the Wisconsin Department of Health Services (DHS), the state agency that runs the EVV program.

Clock in. Care. Clock out. That's it.

Once it becomes a habit, EVV takes seconds. And if anything ever goes wrong, one call to the office fixes it.
