THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Who we are
Avida Personal Care ("Avida") provides home-care coordination and personal-care worker services in Wisconsin. This Notice applies to protected health information ("PHI") we create or receive about our clients.
Our commitment
We are required by law to maintain the privacy of your PHI, provide you with this Notice, and follow the terms of the Notice currently in effect.
How we may use and disclose your PHI
- Treatment. We may share your information with caregivers, nurses, physicians, and other providers involved in your care.
- Payment. We may share information with Medicaid, IRIS, Family Care, Partnership, or other payers to obtain payment for services.
- Health care operations. We may use PHI for quality assurance, caregiver training, credentialing, and internal auditing.
- As required by law. We may disclose PHI when required by federal, state, or local law, including reporting suspected abuse, neglect, or exploitation.
- Public health and safety. We may disclose PHI to prevent a serious threat to health or safety.
- With your written authorization. Other uses and disclosures — including most marketing and any sale of PHI — require your written authorization, which you may revoke at any time.
Your rights
- Request access to and copies of your health information.
- Request an amendment if you believe information is incorrect.
- Request an accounting of certain disclosures we have made.
- Request restrictions on how we use or disclose your information (we will consider requests but are not required to agree in every case).
- Request confidential communications (for example, contact you only by phone).
- Receive a paper copy of this Notice on request.
- File a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. You will not be retaliated against for filing a complaint.
Changes to this Notice
We may change this Notice at any time. Changes apply to all PHI we maintain. The revised Notice will be posted on this page with a new "Last updated" date.
