How it works

Fill out the form above with their name, a photo, and what makes them amazing. One entry per person — make it count!
2. Tag us on Facebook
Post about your nominee and tag @avidahealth. A photo and a Facebook tag are required to win, so don't skip this step.
3. Winner announced Oct 1
The caregiver with the most entries wins $100 and special recognition — and if you nominated them, you get $25.
The fine print
- One entry per person. Each person may submit a single nomination using their own email or phone number.
- A photo of the nominee and a Facebook tag of @avidahealth are required for a nomination to be eligible to win.
- The winning caregiver is the one with the most eligible entries when nominations close September 30 at 11:59 PM.
- The winner will be announced October 1 on our Facebook page and contacted directly.
- Insurance is not required to nominate or to win.
- Void where prohibited.
- Prizes: $100 to the selected caregiver, $25 to the person who nominated them.

Why we celebrate Milwaukee caregivers
Caregiving in Milwaukee rarely looks like a job description. It's the daughter in West Allis who drives across town every morning before work. It's the home health aide in Wauwatosa who notices a change in appetite before anyone else does. It's the husband in Oak Creek learning safe transfers so his wife can stay in the home they built together. Most of that work happens quietly, without applause.
This contest is our way of turning the volume up on it. Every September we ask families, neighbors, and coworkers across Milwaukee County to name one caregiver who deserves to be recognized — whether they work for a home care agency in Milwaukee or simply show up for someone they love.

Caregiver contest questions
Who can I nominate for the caregiver contest?
Anyone who cares for someone else — a professional caregiver at Avida or another Milwaukee agency, a home health aide, a CNA, or a family member caring for a parent, spouse, or child at home. They do not have to be an Avida caregiver or client.
How do I enter the September caregiver contest?
Fill out the nomination form on this page with your nominee's name, a photo of them, and a short story about what makes them amazing. Then post about them on Facebook tagging @avidahealth and paste the link to your post in the form. A photo and a Facebook tag are both required to win.
When does the contest close and when is the winner announced?
Nominations close September 30 at 11:59 PM Central. The winner is announced October 1 on our Facebook page, and we contact the winner and the person who nominated them directly.
What does the winning caregiver receive?
The selected caregiver receives $100 and special recognition from the Avida team. If your nominee is chosen, you receive $25 as a thank-you for celebrating them.
How is the winner chosen?
The caregiver with the most eligible entries when nominations close wins. An entry is eligible when it includes a photo of the nominee and a link to a Facebook post tagging @avidahealth.
Can I enter more than once?
No — one entry per person. Each person may submit a single nomination using their own email or phone number. Encourage friends and family to nominate the same caregiver instead.
Do I need insurance or have to be an Avida client to enter?
No. Insurance is not required to nominate or to win, and you do not need to be an Avida client. Void where prohibited.
Does the caregiver have to live in Milwaukee?
The contest is open to caregivers across Milwaukee and the surrounding communities we serve, including West Allis, Wauwatosa, Greenfield, Oak Creek, Cudahy, and the rest of Milwaukee, Waukesha, Racine, Kenosha, Ozaukee, and Washington counties.
