About the role
Avida Personal Care is a bilingual, Medicaid-certified personal care agency serving seniors, adults with disabilities, and children with autism across Southeast Wisconsin. Our PRN Registered Nurse provides the clinical oversight our clients and Personal Care Workers rely on — on a flexible schedule that fits around your other commitments. Visits typically include new-client assessments, care-plan reviews, and supervisory visits with caregivers.
Responsibilities
- Complete initial in-home nursing assessments for new personal care clients, including the Wisconsin Personal Care Screening Tool (PCST) where required.
- Develop, review, and update individualized plans of care based on each client's physician orders, needs, and preferences.
- Conduct required supervisory home visits (at least every 60 days, or as program rules require) to observe Personal Care Workers (PCWs) and confirm care is delivered as planned.
- Train and orient PCWs on client-specific tasks, safe transfers, infection control, and documentation.
- Reassess clients after hospitalizations, falls, or changes in condition, and update the care plan promptly.
- Communicate with physicians, case managers (Family Care, IRIS, Partnership), and families about client needs.
- Document visits, assessments, and care-plan changes accurately and on time in our records system.
- Recognize and report suspected abuse, neglect, or safety concerns according to Wisconsin requirements.
- Support compliance with Wisconsin DHS 105.17 personal care agency rules and Medicaid documentation standards.
Qualifications
- Active, unrestricted Wisconsin RN license (or multistate compact license valid in Wisconsin).
- At least 1 year of RN experience; home care, community health, or long-term care preferred.
- Current CPR certification.
- Valid driver's license, reliable transportation, and auto insurance for home visits.
- Ability to pass a caregiver background check.
- Strong assessment, documentation, and communication skills.
- Bilingual English/Spanish is a plus, not required.
