Am I a caregiver?
Great question! Many times, we see ourselves as just a spouse, partner, parent, child, sibling, grandchild, friend, etc. helping someone. However, according to the Centers for Medicare and Medicaid Services caregivers are defined as family members, friends or neighbors who provide assistance to a person with a chronic illness or disabling condition. If this sounds like what you are doing—you are a caregiver.
How much do your programs cost?
Our programs are free! They are covered under a health plan or through a state Medicaid program at no cost to you.
Can I get paid to be a caregiver and how much money could I receive?
Yes, you can be paid for the care you give if you qualify for Structured Family Caregiving. The amount of pay is determined by each state’s individual program requirements and the complexity of care needed. Eligible caregivers may earn up to $3,190 per month. What services does Careforth provide?
Careforth offers multiple programs, Structured Family Caregiving and Caregiver Coaching. Regardless of program, Careforth surrounds you with a team of expert coaches, a personal plan, and connects you to local caregiving resources in the community. If you qualify for Structured Family Caregiving, you can also get paid for the care you give.
Do you offer Structured Family Caregiving and Caregiver Coaching in every state?
Currently, Careforth offers Structured Family Caregiving in the following states: Connecticut, Georgia, Indiana, Louisiana, Massachusetts, North Carolina, Ohio, Rhode Island and South Dakota.
We offer Caregiver Coaching through select health plan partnerships and in the state of Indiana.
We also offer Structured Family Caregiving in Pennsylvania through UPMC.
How do I know if I qualify for your program?
If the person you care for is enrolled in a state Medicaid program, you may qualify for Structured Family Caregiving. The best way to know whether you qualify is to give us a call or complete an initial intake form.

We have designed our app with you in mind. In fact, 91% of our caregivers find the app easy to use.
Can I get access to the Careforth App?
You must be enrolled in our program to use the Careforth App. Give us a call to get started. Our app is available for download at the Google Play Store and the Apple App Store.
Are there other names for Structured Family Caregiving?
Structured Family Caregiving is known by many different names across states. It is also known as Adult Foster Care in Massachusetts, Adult Family Living in Conneticut, RIte @ Home Shared Living in Rhode Island, Monitored In-Home Caregiving in Louisiana, Shared Living in Ohio, CarePlus in Pennsylvania, and Coordinated Caregiving in North Carolina.
